Community Health Improvement Plan

Community Health Improvement Plan (CHIP) is a comprehensive plan for improving the health and quality of life of people who live, work, and play in Hartford. It was developed by Health and Human Services Department in collaboration with other agencies and organizations, which helped determine the plan’s priorities, goals, and strategies. The CHIP provides guidance to the department, its partners and stakeholders.
